The Site is owned and operated by Skyline Dental (“Skyline Dental,” “we,” “us,” or “our”).

By accessing, browsing, or using the Site, submitting information through the Site, requesting an appointment, making an online payment, or otherwise using any feature of the Site, you agree to be bound by these Terms and by our Privacy Policy. If you do not agree to these Terms, do not use the Site.

1. Purpose of the Site

The Site is provided for general informational and convenience purposes. It may include information about Skyline Dental, our dental professionals, office location, services, appointment requests, payment options, educational dental content, and other practice-related resources.

The Site is not intended to provide medical, dental, legal, financial, or other professional advice. Information on the Site should not be used as a substitute for an in-person examination, diagnosis, treatment recommendation, or consultation with a qualified dental professional.

No content on the Site creates a dentist-patient relationship. A dentist-patient relationship is established only when Skyline Dental accepts you as a patient and appropriate professional services are provided in accordance with applicable law and office procedures.

If you are experiencing a medical or dental emergency, call 911 or seek immediate emergency care. Do not rely on the Site, an appointment-request form, email, text message, or other online communication for emergency care.

2. Privacy

Our collection, use, and disclosure of Personal Information through the Site is governed by our Privacy Policy, which is incorporated into these Terms by reference.

If you submit information through the Site, you agree that Skyline Dental may handle that information as described in the Privacy Policy and as otherwise permitted or required by applicable law.

If you are or become a patient of Skyline Dental, certain health information may be governed by our separate Notice of Privacy Practices and applicable healthcare privacy laws, including HIPAA, rather than solely by these Terms or the Website Privacy Policy.

3. Informational Content and No Guarantees

We strive to keep the Site accurate, current, and useful. However, dental services, provider availability, office hours, insurance participation, treatment options, fees, promotions, technology, financing availability, and other information may change without notice.

We do not guarantee that:

  • The Site or its content will always be accurate, complete, current, or error-free.

  • Any dental procedure, treatment, outcome, or service described on the Site will be appropriate, available, or suitable for every individual.

  • Any provider, appointment time, service, promotion, or payment option will be available at a particular time.

  • The Site will be available, uninterrupted, secure, or free of viruses or other harmful components.

  • Information provided through an online form will be received, reviewed, or responded to immediately.

Treatment decisions depend on an in-person evaluation, your health history, diagnostic information, clinical judgment, and other relevant circumstances. Individual results may vary.

“Skyline Dental,” associated logos, and other practice names, designs, and marks used on the Site may be trademarks or service marks of Skyline Dental or their respective owners.

Subject to these Terms, Skyline Dental grants you a limited, non-exclusive, non-transferable, revocable license to access and use the Site solely for personal, non-commercial, lawful purposes.

You may not, without our prior written permission:

  • Copy, reproduce, republish, distribute, display, modify, transmit, create derivative works from, or otherwise exploit Site content for commercial purposes.

  • Use Skyline Dental’s name, logos, trademarks, photographs, or other branded materials in advertising, social-media posts, metatags, search-engine keywords, or other promotional materials.

  • Remove or alter copyright, trademark, or proprietary-rights notices from Site content.

  • Use Site photos, videos, patient testimonials, provider bios, or educational content in a manner that falsely suggests an endorsement, affiliation, or relationship with Skyline Dental.

  • Use automated tools, bots, spiders, scrapers, data-mining tools, or similar methods to access, collect, copy, monitor, or extract information from the Site, except as permitted by a generally available search engine.

All rights not expressly granted in these Terms are reserved.

5. Acceptable Use

You agree to use the Site lawfully, responsibly, and only for its intended purposes. You may not:

  • Use the Site in violation of any applicable federal, state, local, or international law or regulation.

  • Submit false, misleading, fraudulent, defamatory, threatening, harassing, unlawful, or otherwise inappropriate information.

  • Impersonate Skyline Dental, a member of our team, another patient, or any other person or entity.

  • Attempt to gain unauthorized access to the Site, servers, databases, accounts, or networks connected to the Site.

  • Interfere with, disrupt, damage, overload, disable, or impair the operation, security, or availability of the Site.

  • Upload, send, or introduce viruses, malware, ransomware, spyware, Trojan horses, worms, corrupted files, or other harmful code.

  • Attempt to circumvent Site security features, access controls, or technical limitations.

  • Use the Site to transmit unsolicited commercial messages, spam, or other unauthorized communications.

We may investigate suspected violations of these Terms and may suspend or restrict access to the Site if we believe a violation has occurred.

6. Appointment Requests and Communications

The Site may allow you to request an appointment, submit an inquiry, call or text our office, or communicate with Skyline Dental through online forms or other electronic methods.

Submitting an appointment request does not guarantee that an appointment has been scheduled. An appointment is not confirmed until Skyline Dental communicates confirmation to you.

You are responsible for providing accurate and current contact information. If you provide a phone number or email address, you authorize Skyline Dental to use that information to respond to your request, communicate about an appointment, provide requested information, and communicate about related administrative matters, subject to applicable law and your communication preferences.

Do not use general website forms, email, or text messages to send urgent requests or time-sensitive emergency information. If you have a dental emergency, call 911, seek emergency care, or call the office directly at 541-389-4807.

7. Online Payments

The Site may provide access to online payment options for patient convenience. Payments may be processed through one or more third-party payment processors.

By making an online payment, you represent that you are authorized to use the selected payment method and that the information you provide is accurate and complete.

Skyline Dental may not receive or store your complete payment-card information. Payment processing is subject to the terms, privacy policies, and security practices of the applicable third-party processor.

An online payment does not alter the terms of your treatment, financial agreement, insurance obligations, or any separate written agreement you may have with Skyline Dental. If there is a conflict between these Terms and a patient financial agreement or other signed agreement with Skyline Dental, the signed agreement will control.

8. Third-Party Websites and Services

The Site may contain links to third-party websites, services, platforms, maps, payment portals, social-media pages, scheduling tools, patient portals, or other resources. These links are provided for convenience only.

Skyline Dental does not control, endorse, monitor, or assume responsibility for third-party websites, services, content, security, availability, privacy practices, products, or transactions.

Your use of third-party websites and services is at your own risk and is governed by the applicable third party’s terms, policies, and practices. You should review those terms and policies before providing information or engaging in a transaction with a third party.

9. Testimonials and Before-and-After Content

The Site may include patient testimonials, reviews, photographs, videos, treatment descriptions, or before-and-after images. These materials reflect individual experiences and are not guarantees or promises of similar results.

Dental outcomes vary from person to person based on factors that may include oral health, anatomy, treatment needs, adherence to care instructions, lifestyle factors, and other individual circumstances.

Any photographs, videos, reviews, or testimonials appearing on the Site are used with appropriate permission or as otherwise permitted by law.

10. Disclaimer of Warranties

To the fullest extent permitted by applicable law, the Site and all information, content, tools, materials, functionality, and services made available through the Site are provided on an “as is” and “as available” basis.

Skyline Dental makes no representations or warranties, express or implied, including warranties of accuracy, completeness, merchantability, fitness for a particular purpose, title, non-infringement, security, availability, or error-free operation.

Without limiting the foregoing, Skyline Dental does not guarantee that:

  • The Site will be uninterrupted, timely, secure, or available at all times.

  • The Site will be free of errors, defects, viruses, malware, or other harmful components.

  • Any errors or defects will be corrected.

11. Limitation of Liability

To the fullest extent permitted by applicable law, Skyline Dental and its owners, dentists, hygienists, employees, contractors, agents, affiliates, licensors, suppliers, and service providers will not be liable for any indirect, incidental, special, consequential, punitive, or exemplary damages arising from or related to:

  • Reliance on information available through the Site.

To the extent liability cannot be excluded under applicable law, Skyline Dental’s total liability for claims arising from or related to the Site will not exceed the greater of:

  1. The amount you paid directly to Skyline Dental specifically for use of the Site during the 12 months before the event giving rise to the claim; or

  2. One hundred dollars ($100).

Some jurisdictions do not allow certain exclusions or limitations of liability. In those jurisdictions, the above limitations apply only to the extent permitted by law.

Nothing in these Terms limits liability that cannot legally be limited or excluded.

12. Indemnification

To the extent permitted by applicable law, you agree to defend, indemnify, and hold harmless Skyline Dental and its owners, dentists, employees, contractors, agents, affiliates, licensors, suppliers, and service providers from and against claims, liabilities, damages, losses, costs, and expenses—including reasonable attorneys’ fees—arising out of or related to:

  • Your violation of these Terms.

  • Your violation of applicable law or the rights of another person or entity.

13. Changes to the Site or Terms

Skyline Dental may modify, update, suspend, discontinue, or restrict access to any part of the Site at any time, with or without notice.

We may revise these Terms from time to time. When we do, we will post the updated Terms on this page and change the Last Updated date at the top of the page.

Your continued use of the Site after revised Terms are posted means that you accept the revised Terms. If you do not agree with the revised Terms, you must stop using the Site.

14. Governing Law and Venue

These Terms and your use of the Site are governed by the laws of the State of Oregon, without regard to its conflict-of-law principles.

Any dispute arising from or relating to these Terms or the Site that is not otherwise subject to a valid, enforceable written agreement between you and Skyline Dental will be brought exclusively in the state or federal courts located in Deschutes County, Oregon, unless applicable law requires otherwise.

You consent to the personal jurisdiction and venue of those courts.

15. Severability and Waiver

If any provision of these Terms is held to be invalid, unlawful, or unenforceable, that provision will be enforced to the maximum extent permitted by law, and the remaining provisions will remain in full force and effect.

Skyline Dental’s failure to enforce any provision of these Terms does not waive our right to enforce that provision or any other provision at a later time.

16. Entire Agreement

These Terms, together with the Privacy Policy and any other terms, notices, patient agreements, financial agreements, consent forms, or policies that apply to a specific Skyline Dental service or transaction, constitute the agreement between you and Skyline Dental regarding your use of the Site.

If there is a conflict between these Terms and a separate written agreement you sign with Skyline Dental, the separate written agreement will control to the extent of the conflict.
